* Eligibility. For one chance to win, fill in completely, seal, and mail this form. Alternatively, for five chances to win, register your product online at CardiacScience.com/iPod. Open only to purchasers of Cardiac Science products. Not open to employees or immediate family members of employees of Cardiac Science Corp., its affiliates, subsidiaries, or partners; federal, state, county, municipal, or other government employees; or healthcare practitioners licensed in the State of Massachusetts. The odds of winning depend on the number of eligible entries received, but are unlikely to be less than 1 in 500. Customers may enter only once for each unit of product purchased. This drawing period ends in October 2010 but may be extended at Cardiac Science's option. Prize. The following prize will be awarded: an iPod shuffle, $49.99 approximate retail value. Winner is responsible for paying all costs and expenses related to the prize that are not specifically mentioned, including but not limited to taxes (including income taxes). Prize Drawing. Prize will be awarded once at the end of each month during the drawing period in a random drawing. We will notify the winner by e-mail or certified mail within seven business days of drawing. If the prize is not claimed within seven business days of notification, or if winner does not meet eligibility criteria, an alternate winner will be selected.
